Here's everything you need to know before your first visit to Polo Dental.
Your first visit
Plan for 60–90 minutes. You'll meet Dr. Zadov, complete a comprehensive exam, and leave with a clear plan tailored to your goals.
New patient forms
We'll email your intake forms in advance so your first visit is spent talking, not typing.
What to bring
Photo ID, insurance card, list of medications, and any prior dental records if available.
Comfort options
Noise-cancelling headphones, warm blankets, nitrous oxide, and oral sedation — whatever helps you feel at ease.
Sedation options
Nitrous oxide and prescribed oral sedation are available for patients who prefer additional relaxation.
Cancellation policy
We ask for 48 hours notice on schedule changes so we can offer the appointment to another patient.
Records & privacy
We follow HIPAA and only share your records with providers you authorize.
